The aim of this study was to adapt the “Children's Leisure Activities Study Survey” developed by Telford, Salmon, Jolley, and Crawford in 2004, for Turkish language and culture and to determine its validity and reliability. The participants of the study consisted who volunteered of 405 students aged 11 to 14 attending secondary school in Ankara in the 2017-2018 academic year. In the process of adapting the questionnaire, a pilot study was conducted to translate from English to Turkish, to translate from Turkish to English, to test the intelligibility of Turkish expressions, to remove some activities, to add some activities and to test the intelligibility of the questionnaire. Afterward, a one-week Physical Activity Diary was applied to 104 students for validity study. At the end of the 7th day, the physical activity diary was collected from the students. And then the questionnaire was applied to students. For the reliability study, the questionnaire was applied to 301 students twice a week apart. To determine the validity, intraclass correlation coefficients were calculated, and the (inactive) part of the questionnaire with low intensity activities was found not valid (p> 0.05), and the (active) part of the moderate intensity, vigorous intensity and moderate to vigorous intensity physical activities was found valid. Intraclass correlation coefficients ranged between 0.50 and 0.95. As a result of the reliability analysis, it was found that the intraclass correlation coefficients ranged between 0.52 and 0.91 for all age levels (11-14) in different types of activities. As a result; The Children's Leisure Activities Study Survey (CLASS) cannot be used to evaluate low-intensity activities in children aged 11-14, but it is a valid and reliable questionnaire for the assessment of moderate intensity, vigorous intensity and moderate to vigorous intensity physical activities.
