Mandibular Gömülü Üçüncü Molar Dişlerin Çekimi Sonrası Zorluk Derecesine Göre Gelişen Komplikasyonların ve Hasta Memnuniyet Derecelerinin Birlikte Değerlendirilmesi
Abstract
The extraction of mandibular impacted third molar teeth is a commonly performed procedure in the field of dental and maxillofacial surgery. Depending on the degree of the complications which occur after removal of mandibular third molar teeth may affect the quality of life adversely in the postoperative period. Therefore, determining the difficulty degrees of impacted teeth have a particular importance. The aim of this study is to determine the relationship between the difficulty degree of mandibular impacted third molar teeth removal, the complications which occur after removal and patient satisfaction. A total of 150 patients who were between the ages of 15 and 51 years were included in our study. Of them, 99 were female and 51 were male. Preoperatively, the data of patients such as age, gender, systemic condition, bad habits, oral hygiene, mouth opening and classification of mandibular impacted third molar teeth were recorded. Also, the duration of operation was recorded in the same format during the operation. Seven point Likert-type scale was used to determine the degree of satisfaction and complication. The patients filled in the grading form of satisfaction and complication on the postoperative 7 day after the stiches were removed and their mouth openings were measured with callipers. The statistical analysis of the data that were obtained in our study was performed using the tests of the IBM SPSS Statistics for Windows version 23 package program and the value of p <0.05 was accepted as significant. In our study, it was determined that as the difficulty degree of impacted teeth increased, the duration of operation prolonged and the frequency of trismus increased and this result was found to be statistically significant. The measurements of mouth opening confirmed this data. Also, the differences between the duration of operation and the patient satisfaction scores on the subjects of edema, changes in the diet, impact on social activities, and disappearance of preoperative complaints were also found to be statistically significant. There was also a statistically significant relationship between the smoking associated pain and the impact on daily activities due to this pain. Also, a significant relationship was detected between the age of the patients and the changes in the diet, impact on social activities and taking time off work. The difficulty in eating food, changes in voice, difficulty of speaking, incidence of fatigue and ecchymosis were also found to be significantly higher in the females compared to male patients. Also, the female patients stated that the changes after surgery were more negative than their expectations when compared to the males. A statistically significant difference was found between the difficulty degree of impacted teeth, pain and the efficacy of painkillers. As the degree of difficulty increased, patient satisfaction was detected to decrease. As the most important finding, a statistically significant relationship was found between the difficulty degree of impacted teeth, complication and patient satisfaction.
