Serum Adiponektin Seviyesi ile Uveal Melanom Riskinin ve Prognozda Etkili Olan Klinik ve Histopatolojik Bulguların İlişkisi

The relation between serum adiponectin levels with the risk of uveal melanoma and clinicopathological findings associated with the prognosis, Hacettepe University, Faculty of Medicine, Thesis in Ophthalmology, Ankara, 2014. The objective of this study was to analyze the association between serum adiponectin level and uveal melanoma risk as well as various clinicopathological characteristics. Uveal melanoma is the most common primary intraocular neoplasm in adulthood and metastasis causes a high mortality. Because serum insulin like growth factor (IGF)-1 levels, known to have a key role in metabolic syndrome, have been proven to be associated with uveal melanoma, we hypothesized that decreased adiponectin levels, also shown to be associated with metabolic syndrome and increased malignancy of various cancers, might also have a potential association with uveal melanoma. In this study we investigated serum adiponectin levels in 86 patients with uveal melanoma, 38 subjects with choroidal nevi, and 86 healthy individuals. The body mass index, age and the gender of the groups were similar. Patients with uveal melanoma were stratified into 3 groups according to COMS size classification for uveal melanoma. Subjects with choroidal nevi were stratified into two groups based on their clinical risk factors as low risk and high risk. Uveal melanoma group had higher HOMA scores than choroidal nevi group or controls (p<0.001). Serum adiponectin levels were significantly lower in the uveal melanoma group and choroidal nevi group than controls (p<0.001 for both). Serum adiponectin levels of uveal melanoma group and choroidal nevi group were similar (p= 0.390). Additionally, the serum adiponectin levels were significantly lower in the metastatic uveal melanoma group than in the non-metastatic group (p= 0.018). Serum adiponectin levels were similar among COMS 1, COMS 2 and COMS 3 groups (p=0.170). There was no statistically significant difference between the low risk and high risk choroidal nevi groups in terms of serum adiponectin levels (p=0.508). The results of the present study imply that decreased serum adiponectin levels are associated with the risk of uveal melanoma, choroidal nevi and metastasis.
