Medikal Onkoloji Polikliniğinde Tedavi Alan Hastalarda Hedefe Yönelik Tedavi ile İlgili Sorunların Belirlenmesi ve Önlenmesinde Klinik Eczacının Rolü

Cancer is defined as uncontrolled growth of cells, which can invade and spread to other organs and tissues of the body. The primary goal in cancer treatment is to eradicate the cancer. Cancer treatments can be classified as surgery, radiation, chemotherapy and biological therapy. The aim
of this study is to determine, reduce and prevent drug therapy problems with targeted therapies in outpatient oncology clinic by the involvement of clinical pharmacist within a multidisciplinary healthcare team. According to the purpose of the study patients who receive bevacizumab, cetuximab, panitumumab, nivolumab, ipilimumab or pembrolizumab treatments were considered to be eligible. Fifty six patients who receive targeted therapies between October 2015 and March 2016 at the Hacettepe University Oncology Hospital Oncology Outpatient Clinic were included. A total of 274 pharmacist's follow-ups were completed for 54 patients. According to the PCNE V7.0 classification system, a total of 105 drugs related problems were determined and 149 recommendations were proposed, of those 92 recommendations were accepted giving the acceptance rate as % 95.65. Among 105 drug related problems, 68 were completely resolved. According to results of the study, drug related problems can be clearly determined and patient's drug treatment can be optimized by the active involvement of comprehensively skilled clinical pharmacist in monitoring process. For forthcoming studies, it can be suggested that patient‟s quality of life and treatment effectiveness can be improved in terms of time and cost by the development of a new model for patient care/treatment monitoring process.
