In this study, it was aimed to determine the effects of patient, disease, treatment and social factors on the treatment burden in adults with chronic obstructive pulmonary disease and to determine the effects of treatment burden on health outcomes (quality of life, mortality risk, treatment adherence), resource use (outpatient, inpatient and emergency admissions and imaging, laboratory and pulmonary function tests) and the effect on the cost of the ilness, both directly and indirectly. The sample of this cross-sectional study, which was carried out between 05/04/2019-08/02/2020, consisted of 332 patients with chronic obstructive pulmonary disease who applied to Kırıkkale Yüksek İhtisas Hospital Pulmonary Diseases Outpatient Clinic. In the collection of research data, primary data was used through a questionnaire, and secondary data was used through patient files and invoices. Validity and reliability analyses, correlation analysis and structural equation modeling were used to analyze the research data. As a result of the analyzes, it is seen that the scales in the research model are valid and reliable; age and self-efficacy level from the characteristics of the patient, COPD stage and comorbidity level from the characteristics of the disease, the dosage form and the additional instructions regarding the treatment regimen from the characteristics of the treatment, and the level of perceived social support from the characteristics related to the social factors were a significant effect on the treatment burden. It was determined that the treatment burden had both direct and indirect effects on all health outcomes and the number of pulmonary function tests of resource use variables, but had no effect on other resource use variables and the cost of the illness. It is thought that the results obtained from the research will provide both health administrators and health care providers with important evidence-based information in the management of chronic obstructive pulmonary disease.
