Arteria Ilıaca Interna’nın Dallanma Paterninin Radyolojik Yöntemle Sınıflandırılması
Özet
ABSTRACT
The internal iliac artery is the main artery that is supplying the pelvis. The artery, which has numerous variations, is important, not only for anatomists, but also important for surgeons and radiologists. Since it is clinically important, it has been subject of many studies. These studies purposed to increase the success and to decrease the complications of the interventional approach the internal iliac artery, by simplifying and grouping the variations of the vessel under the title of “branching pattern”. Most of these studies are cadaveric studies. A few number of these studies are radiological study. These radiological studies have limited number of cases or are only around male population. The anatomy of the internal iliac artery is very important for choosing right equipments and decreasing complications in the interventions or surgical procedure including this region. The aim of this study is to enlighten the radiological anatomy of the internal iliac artery. In this study the branching pattern types of the internal iliac artery is determined in 249 patient, by computerized tomography angiography imaging, according to the Adachi classification. Additionally the length between the aortic bifurcation to the origin of the internal iliac artery (the length of the common iliac artery) was measured in each pelvic halves. Type 1 branching pattern is determined in 66.1%, Type 2 in 24.5%, Type 3 in 9.2%, Type 4 in 0.2% of the cases. Type 5 branching patterns are not determined in any of the cases. These ratios are important for setting the right shaped vascular graft (for example bifurcated grafts). Furthermore, the knowledge of the structure that the practitioner will face during the selective interventions of the artery is important. In the comparison between pattern types according to the sex and age groups; or right and left sides there is no statistically significant difference between groups. The length of the common iliac artery varies between 2.19-11.77 cm. These values are important for aneurysm repair operations, while deciding the type, length and setting technic of the vascular graft.
