Risk Değerlendirmesinde FMEA Yöntemine Bulanık Mantık Yaklaşımı: Deney ve Kalibrasyon Laboratuvarları Uygulaması

Within the scope of the thesis, it is aimed to carry out a risk analysis with Failure Mode And Effect Analysis (FMEA) and Fuzzy Failure Mode And Effect Analysis methods in order to determine the areas that could pose a risk to laboratories within the framework of risk-based thinking approach which is a relatively new approach for test and calibration laboratories. The aim of this study is to compare the results obtained with Failure Mode And Effect Analysis and Fuzzy Failure Mode And Effect Analysis methods and to evaluate the performance of the Fuzzy logic approach to Failure Mode And Effect Analysis method. At this purpose, it was examined the non-conformities identified in 91 assessments conducted by the Turkish Accreditation Agency in the test and calibration laboratories according to the ISO/IEC 17025:2017 General Requirements For The Competence Of Testing And Calibration Laboratories between October-March 2019 and identified 199 error types. The inputs of Occurrence, Severity and Detection were evaluated by 5 experts who have knowledge and experience in the related subject and a common input was obtained by taking the geometric mean of the evaluations for each parameter. The data of risks were prioritized by using Failure Mode And Effect Analysis and Fuzzy Failure Mode And Effect Analysis method and the results obtained by classical and fuzzy Failure Mode And Effect Analysis were compared. Due to the high correlation coefficient, the order of priority based on the Fuzzy Risk Priority Number calculation is largely overlapping with the Risk Priority Number sequence. Therefore, the assessment of risk using linguistic variable(s) is considered to be substantially parallel to the situation where the inputs of the risk calculation are quantitative.
