Occupational health and safety is one of the most important issues in the world and in our country under the heading of social policy and employment, and human and economic losses caused by problems related to this issue negatively affect all countries. Occupational health and safety has become one of the priorities of developed countries, especially as the negative effects of the losses caused by problems related to this issue, which are reflected in all countries, continue to be below the expected results despite all efforts. In order to improve occupational safety and health, it is necessary to prevent accidents at work and work-related deaths. For this reason, it is very important to identify the causes of accidents.. In this study, which was conducted as a record-based descriptive research, the records of fatal occupational accidents of the Ministry of Labour and Social Security were examined. This study aims to investigate the causes of fatal occupational accidents that occurred in Turkey between 1 January 2019 and 31 October 2022, to categorise fatal accidents that cannot be fully attributed to a cause, and to classify them according to their severity. In this context, a total of 3936 fatal occupational accident records that occurred between 2019-2022 were analysed. Of the occupational accidents within the scope of the study; 26.7% occurred in the construction sector, 20.6% in the manufacturing sector, 19.1% in the transportation and storage sector. When the causes of the accidents were examined by Pareto analysis, five causes of accidents were determined. While the rate of accidents coded as uncertain was 31.4%, the uncertainty rate was reduced to 6.8% with the new classification. Risk assessment and incident analysis processes should be activated to prevent occupational accidents. Accident data systems should be encouraged to be accurate, complete, and aligned with international standards.
