Sleep disturbances are more commonly observed in patients with ulcerative colitis compared to the general population. Not only during active disease but also during periods of remission, the quality of sleep and sleep patterns can be disrupted. The aim of this study is to assess sleep quality in patients with ulcerative colitis, identify potential pathologies that may cause sleep disturbances and reveal associated clinical conditions. The study included ulcerative colitis patients who applied to the Department of Gastroenterology at Hacettepe University between June and July 2023 and a healthy control group with no chronic illnesses. Ulcerative colitis patients were grouped according to the partial Mayo score. The study included 22 active, 28 remissive ulcerative colitis patients and 24 healthy controls. The participants' sleep quality was evaluated using a questionnaire that included the Pittsburgh Sleep Quality Index (PSQI). Individuals with a Pittsburgh global sleep score of 5 or higher were considered to have poor sleep quality. The mean age at diagnosis for the ulcerative colitis patient group was determined as 34.22±13.92 years. Intestinal symptoms and findings were more pronounced in active ulcerative colitis patients, while there was no significant difference in extraintestinal symptoms between the groups. According to the evaluation of sleep quality among the groups, it was found that active and remissive patients had worse sleep quality compared to the control group (p<0.05). Based on the ROC analysis, when the cut-off value for the PSQI global score was set at (>6.50), it was determined that the PSQI showed disease activation in active ulcerative colitis patients with a sensitivity of 59.1% and specificity of 32.7%. Furthermore, a significant relationship was found between the metabolic syndrome scores evaluated in the active ulcerative colitis group and sleep quality (p<0.05). In conclusion, it was determined that sleep quality is impaired in the ulcerative colitis patient group regardless of disease activity and the PSQI global score can be used as a diagnostic decision-making parameter for predicting ulcerative colitis disease activation.
