dc.description.abstract | Obstetric patients are under much risk compared to normal population in terms of difficult airway incidence. In cases of hypertension accompanied pregnancies like preeclampsia, this risk increases even more. In case of difficult or unsuccessful ventilation and intubation, there could be life threatening cases for maternal and fetal life. In this patient group with a difficult airway expectancy, pre-op detailed airway examination is crucial. In this study, we aimed to compare Mallampati Classification which is used in difficult airway examination, thyromental distance, neck circumference and with the help of ultrasound (USG) skin to hyoid distance, skin to epiglottis distance and anterior measurement of tongue base and also Optic Nerve Sheath Diameter (ONSD) in preeclampsia diagnosed (n:20) and normal pregnant woman with no internal disease (n:20), during c-section and at postoperatif 48th hours. 40 Patients who are in ASA II-III group, have single pregnancy are included to this case control and prospective study. Study is conducted with an anaesthetic assistant with 4 years of experience. At the end of our study both preeclamptic pregnancy group and normal pregnant group; ONSD, Mallampati Score and skin-epiglottis distance were significantly decreased at 48th hour postoperatively compared to intraoperative measurement, but no correlation was shown between the two groups. However, when preeclamptic pregnant women were examined, the skin-hyoid distance was found higher than normal pregnant women. Further clinical experience and studies with sufficient number of patients are needed to evaluate these results in depth. | tr_TR |

dc.description.ozet | Obstetrik hastalar; normal popülasyona göre zor havayolu insidansı açısından daha fazla risk altındadır. Preeklampsi gibi gebeliğe hipertansiyonun eşlik ettiği durumlarda bu risk daha da artmaktadır. Zor veya başarısız ventilasyon ve entübasyon durumunda; maternal ve fetal yaşamı tehlikeye atan olaylarla karşılaşılabilir. Zor havayolu beklentisi olan bu hasta grubunda operasyon öncesi havayolu muayenesinin detaylı yapılması büyük önem taşır. Bu çalışmada; preeklampsi tanısı alan (n:20) ve dahili hastalığı olmayan normal gebelerde (n:20), sezaryen sırasında ve postoperatif 48. saatlerinde; zor havayolu muayenesinde kullanılan Mallampati Sınıflaması, Tiromental Mesafe, Boyun Çevresi ve ultrason (USG) yardımıyla Cilt-Hiyoid Mesafesi, Cilt-Epiglot Mesafesi ve Dil kökünün anterior ölçümünü ve aynı zamanda Optik Sinir Kılıf Çapını (OKSÇ) karşılaştırmayı amaçladık. Bu vaka kontrol ve prospektif olarak planlanan çalışmaya; ASA II-III grubu, tekli gebeliğe sahip, 40 hasta dahil edilmiştir. Çalışma 4 yıllık tecrübesi olan anestezi asistanı ile yapılmıştır. Çalışmamızın neticesinde; hem preeklamptik gebe grubu hem de normal gebe grubunda; OSKÇ, Mallampati Skoru ve cilt-epiglot Mesafesi postoperatif 48. Saatte, intraoperatif ölçüme göre anlamlı olarak azalmış ancak iki grup arasında korelasyon gösterilememiştir. Ama preeklamptik gebeler incelendiğinde cilt-hiyoid mesafesi, normal gebelere göre daha yüksek bulunmuştur. Bu sonuçları daha derinlemesine değerlendirmek için, yeterli hasta sayısına sahip, ileri klinik deneyim ve çalışmalara ihtiyaç vardır. | tr_TR |
