Geriatrik Hastalarda Mini Nütrisyonel Değerlendirme (Mna) Testinin Uzun ve Kısa (Mna-Sf) Formunun Geçerlilik Çalışması
Özet
Early diagnosis and treatment of malnutrition, which is a common disease in geriatric population and associated with increased morbidity and mortality, is quite important. There is no biochemical indicator as a gold standard for malnutrition diagnosis. Thus various methods are used worldwide for screening of malnutrition. The most commonly used one is Mini Nutritional Assessment(MNA) among these methods both in our country and worldwide. There are two forms of MNA: the long and the small one (MNA-SF). In this study, it was intended to perform the validity study of MNA and MNA-SF in our country. Our study included 236 patients (89 male and 147 female) aged 65 years and older, who admitted to Hacettepe University Geriatrics Outpatient Clinic. Four groups of data regarding patients were recorded (anthropometric measures, biochemical indicators, 3-days diet record and geriatric assessment scales). Short and long form of MNAs were performed in patients. Two clinicians blinded to MNA results evaluated the nutritional status of patients by using these four groups of data. The agreement between the results of MNA?s long and short forms was observed by clinician?s evaluations. The long MNA was performed in 214 elderly adults; 55.1% of them were found to be normal, 29.9% as at risk and 15% as malnutrited. 236 elderly adults were assessed by MNA-SF; 48.3% of them were found to be normal, 32.2% as at risk and 19.5% as malnutrited. Agreement between MNA and MNA-SF (kappa agreement 0.75) was at a good level. Sensitivity and specificity of MNA were found to be 92% and 86%, respectively while sensitivity and specificity of MNA-SF were found to be 94% and 81%, respectively. Agreement between MNA?s long and short forms by clinician?s evaluation was found to be good when assessed with kappa statistics (kappa coefficients; 0.68 and 0.66, respectively). As a result, it was suggested in this study that MNA test with long and short forms is a validated method for the screening of elderly adults in Turkey.
