Diyabetik Hastalarda Acil Başvuru ile Eser Elementler Arası ilişki

The effects of trace elements (zinc (Zn), chromium (Cr), selenium (Se), iron (Fe), copper (Cu), vanadium (Va) etc.) on the glucose metabolism is known. Studies conducted in recent years have tried to prove that they play a role in the etiology of the disorder and can be applied in the concomitant treatments with insulin and oral antidiabetics. Patients who admitted to the Department of Emergency Medicine between January 1, 2012 - February 29, 2012 and who hadpreviously been diagnosed as DM were accepted to examine the relationship between the emergency preference and trace elements. The patient files and automation systems of the hospital were examined. Demographical characteristics, complaints, histories, vital findings, physical examinations of the patients, antidiabetics they use, their blood sugar levels at the time of admissions, their laboratory findings were evaluated. After obtaining written informed consent from the patients, trace element levels in blood samples were measured by atomic absorption spectrophotometric method.. The correlation between trace elements levels based on the diagnoses and complications were evaluated. 151 (84 women) were the study group, and 98 (40 women) were the control group. The mean age of the study group was found to be 63.74 (22-93). 20.5% of patients (n=31) have pneumonia, 11.3 % (n=17) of patients have acute coronary syndrome and 8.6%(n=13) of patients have diabetic ketosis in study group who admitted to the Emergency Service. Trace element levels in the study group (Se: 117.41 mg/dl, Zn: 78.27 mg/dl, Fe: 81.06 mg/dl, Cu: 77.36 mg/dl, Cr: 93.19 nmol/l, Va: 0.81 ng/ml)were compared with the levels of trace elements (Se: 112.51 mg/dl, Zn: 79.41 mg/dl, Fe: 73.91 mg/dl, Cu: 74.38 mg/dl, Cr: 86.50 nmol/l, Va: 0.73 ng/ml) of control group. There was no difference between the levels of trace elements in comparison. We also compared the trace elements levels in patients with complications to the patients without complications. Though there were some differences but none of them reached to statistical significance. We conclude that no relation was found between complications of diabetes and trace elements. Our findings might be due to the small number of patients with complications therefore future studies with large numbers are needed for better conclusion.
