Ani Işitme Kaybı Tedavisinde Kullanılan Steroidlerin İntratimpanik ve Sistemik Uygulamalarının Etkinliklerinin Karşılaştırılması

Sudden hearing loss is a medical emergency which is defined as a sensorineural hearing loss of at least 30 decibels, including at least three consecutive frequencies and occuring within 72 hours. Autoimmunity, viral factors and vascular causes are thought to contribute to the etiopathogenesis. Corticosteroid treatment has been considered the most effective treatment modality so far. Application route of corticosteroids may vary; in the past systemic administration was routinely preferred whereas intratympanic method with low potential side effects and directly affecting the inner ear is much more popular today. This study is aimed to evalute the effectivenesses and advantages of these different application routes. This study is planned as prospective randomized controlled trial, 34 patients are distributed randomly to two groups which are treated with 'oral' or 'intratympanic' corticosteroids. Recovery rate in the 'intratympanic' group was %84.2, whereas in the 'oral' group it was %87.5. The difference between the recovery rates was not statistically significant. These findings support the ideas that intratympanic steroid therapy is an alternative to systemic steroid therapy and is an option in the initial treatment of sudden hearing loss.
