Inhalasyon Anesteziklerinden Sevofluran ve Desfluranın Mikrosirkülasyon Üzerine Karşılaştırmalı Etkileri

Anesthetic agents can cause microcirculatory alterations which may lead to organ dysfunction. During non-cardiac surgery, the effects of anesthetic drugs on microcirculation is not well known. The aim of this study is to determine the effects of sevoflurane and desflurane on microcirculation in ASA I-II patients which undergone ≥ 2 hour surgery in Hacettepe University Hospital Operation Rooms during the period 01/11/2012 - 31/12/2012 by using sidestream dark field imaging technique (SDF). 39 adult patients who provided informed consent were included in this study. Patients were divided in to two groups according to the inhalation anesthetic used: Group I - Sevoflurane group (n=20) and Group II - Desflurane group (n=19). These agents were used in these patients during whole operation period. Hemodynamic parameters and SDF data were recorded after induction and 2nd hour of surgery. The groups were similar for demographic, hemodynamic and laboratory data. In sevoflurane group MFI and PPV values showed an increase by time, whereas TVD and PVD values decreased. Only the increase in PPV values was statistically significant (p<0.05). In desflurane group MFI values increased by time, whereas TVD, PVD and PPV values decreased. But these differences were not statistically significant. In conclusion, during non-cardiac surgery in ASA I-II patients sevoflurane and desflurane do not alter microcirculation significantly.
