Sentrik Bite Blokların Vertikal Düzlemde Dentofasiyal Yapılar Üzerine Etkilerinin Değerlendirilmesi

AKDAŞ, E. K. The vertical plane effects of centric bite blocks on dentofacial
structures. Hacettepe University, Institute of Health Sciences, Speciality Thesis in
Orthodontics Programme, Ankara, 2017. The aim of this prospective study is to
examine the effects of bite blocks prepared in centric relation during the prepubertal
growth period. Subjects were selected from among the patients who were consulted
to Hacettepe University Faculty of Dentistry. For the aims of this study, a group of 16
patients, between 8 and 11 years of age, having vertical growth pattern and open
bite tendency were recruited. Bite blocks were fabricated according to the centric
relation position and hold in place until a positive overbite was achieved (Treatment
time: 9,7±1,2 month). 15 subjects who were under evaluation for orthodontic
treatment, and matched for age were selected as the control group from a previously
performed study. Analyzes were performed on pre and post-treatment lateral head
films in order to define skeletal and dental changes. Data of the treatment and
control groups were assessed using Kolmogorov-Smirnov, Shapiro Wilks, dependent
and independent groups t test. The angular values showing the vertical development
of the face decreased in the treatment group (p <0,05) and increased in the control
group (p> 0,05). The increase in ramus height and posterior face height was
significantly higher in the treatment group (p <0.05). The increase in the anterior face
height in the control group was significantly higher than the treatment group (p
<0.05). Mandibular skeletal measurements showed significant increases in the
treatment group compared to the control group (p <0,05). In the treatment group,
the vertical positions of the upper molar teeth increased less than the control group.
There was a significant increase in overbite in the treatment group (p <0.05). As for
the soft tissue evaluation, it has been observed that pogonion became more
prominent in treatment group (p <0.05). It can be concluded that centric bite blocks
contributed to the treatment of anterior openbite by providing an upward and
forward rotation of the mandible, elongation of the posterior facial height and ramal
height, and restriction of upper molar eruption.
