Özet
When assessing hospitals, drawing conclusions solely based on a single metric such as patient treatment is inadequate in many aspects. In the contemporary world, it is necessary to define hospital spaces from a system approach. According to this approach, hospitals function as a system with a feedback feature, in which the received input are processed in an environment that shifts and varies, and the processed input are returned to the same environment at the end of the process cycle. In this systematic approach the system inputs may be listed as: patients, human resources, materials and financial resources. Competition is said to positively affect the quality of service offered by competing hospitals. Based on this information, it is clear that spatial factors have an impact on the communication between patients and doctors in today's healthcare environments. İt is inevitable that patient satisfaction with and their percevied confidence at the end of the health care process, together with the relation that is formed between the patient and physician are affected by the space where these interactions take place. The sense of place, and the effect of elements that define place on the communication between patient and physician form the main focus points of this thesis.
In the first part of the study; The main purpose, scope, method and fundamental problem of the study are explained.
In the second part of the study; historical development, general characteristics and classification of hospitals in the post-republic period in Turkey are discussed.
In the third part of the study; hospitals and human interaction; psychophysics, human behavioral theories, human needs, culture, patient rights concepts in hospital environments are discussed in relation to patient space.
In the fourth part of the study; interactions with space in hospital settings have been assessed. The relationship between the items in a room, the perception of the room, the laws affecting perception, language of the room, meaning of space, sense of quality of the room and relation with space are discussed. The design of hospital spaces have been analyzed by considering the relation between lighting, acoustics, air conditioning, furniture and accessory elements, materials, color and texture concepts.
In the fifth chapter of the study; human-space interaction in hospitals have been assessed. Communications model, fundamental communication processes, sensation and perception, elements affecting perception, spatial perception, cognition and evaluation, spatial behavior, continuity of user-designer communication, patient satisfaction in hospitals, job satisfaction in healthcare workers and satisfaction in hospitals in relation to space are discussed.
In the sixth part of the study; information on the conduced field work on the effect of space on patient - doctor communication in the examination rooms of Hacettepe University Hospitals’ Plastic, Reconstructive and Aesthetic Surgery department is provided. Definition and scope of the field work, and the method and process of determining the sample size have been discussed. Results of the field study have been evaluated, analysis of survey data have been analyzed and evaluated by statistical methods, and hypotheses have been tested in this chapter.
In the conclusion section of the study; results obtained through data from the field study and the results of the evaluation of the hypotheses of the study are further emphasized and summarized. According to the interpretation of the data obtained by the fieldwork; the physical conditions of the examination rooms were found to be effective to the communication established between the physician and the patient. According to the evaluation of the physical conditions of inspection areas; among other variables affecting physician-patient communication; it is understood that the use of comfortable furniture is the most efficient one. The fact that parents of child patients having more spatial preferences than other patients is another important result of the research. It has been evaluated that place is an effective element in establishing the trust relationship between the patient and the physician in examination rooms.
