Maksiller Molar Dişlerin Vertikal Kontrolünde Kullanılan Transpalatal Arkın Dentofasiyal Yapılar Üzerine Etkilerinin Değerlendirilmesi

The aim of the present study is to evaluate the effects of the transpalatal arch, which is used for maxillary molar vertical control, on dentofacial structure to compare untreated control group and prepubertal patients with Angle Class 1, hyperdivergency. For this purpose, 15 patients with a mean age of 9,85±0,99 years underwent transpalatal arch application. The transpalatal arch was 6 mm away from the palatinal mucosa and include an acrilic button on the Coffin loop. The control group consisted 13 patients with mean age 9,30±0,92 years who had been followed for dentition and growth without any orthodontic treatment. Each subject’s digital lateral cephalometric, panoramic and hand-wrist radioraphs and dental study casts were obtained before and after treatment period. In addition to these records, tongue pressure measurements during swallowing were performed before treatment and at the end of treatment in treatment group. Independent sample T test was used to evaluate intergroup differences and paired sample T test was used to evaluate intragroup diffrences. Cephalometric evalution showed that, maxillary and mandibulary skeletal parameters did not reveal a difference between two groups. Comparing to control group, in treatment group overbite increases significantly was observed. Maxillary molar eruption was restricted and intrusion of maxillary molar with functional forces was occured. The transpalatal arch was an effective appliance for restricting vertical dimension increase. Tongue pressure on transpalatal arch while swallowing is adequate for maxillary molar vertical control.
