Cerrahi Olmayan Periodontal Tedaviyle Birlikte Diode Laser Uygulamasının Etkinliğinin Klinik ve Biyokimyasal Olarak Değerlendirilmesi

Periodontitis is an infectious disease that causes destruction of periodontal tissues with complex etiology that develops due to local and systemic factors. Host-derived enzymes, cytokines and proinflammatory markers are the main elements that play a role in this degradation. New techniques such as non-surgical or surgical methods combined with laser application are used for its treatment. The aim of this study is to evaluate the effectiveness of diode laser application in addition to non-surgical treatment in periodontitis treatment clinically and biochemically and to assess the potential biomarkers for use. The subjects were divided into two groups as “Individuals Applied with Diode Laser in Addition to Non-Surgical Periodontal Treatment” (Laser group, n = 15) and “Individuals with Non-Surgical Periodontal Treatment (SRP)” (Control group, n = 15). While only non-surgical periodontal treatment was applied to individuals in the control group; In the laser group, diode laser was applied in addition to this treatment. All individuals were examined 1 and 3 months after treatment. Clinical parameters (pocket depth, clinical attachment loss, bleeding on probing, gingival index, plaque index) and gingival crevicular fluid (GCF) sampling were obtained at each control. IL-1β, IL-10, IL-17, OPG, RANKL, TWEAK, Sclerostin levels in GCF samples were measured with ELISA method. Clinical parameters showed a significant change in the direction of health over time in both groups. Compared to the control group, in the laser group, at the end of the first month, bleeding on probing values and GCF volume; at the end of third months, GCF volume and clinical attachment loss values decreased significantly. While IL-17, TWEAK and Sclerostin levels in GCF samples showed significant changes in both groups over time; IL-17 level was significantly lower in the laser group compared to the control group at third months. Clinical and biochemical changes in the treatment groups over time have shown that both methods are successful in the treatment of periodontitis. It has been shown that diode laser application in addition to non-surgical periodontal therapy has a limited effect on periodontal healing. It was concluded that IL-17, TWEAK, Sclerostin, which differ significantly in GCF, and IL-1β and RANKL which correlate with clinical parameters, It is valuable in terms of contributing to the goal of using it as a biomarker in the diagnosis of periodontitis.
