Sistemik Hiperlipideminin Tendon Üzerine Etkisinin Shear Wave Elastografi ile Değerlendirilmesi

Recent studies have shown that increased adiposity and hyperlipidemia may cause tendon pathology. The aim of this study was to evaluate the effect of systemic hyperlipidemia on the patellar tendon stiffness by shear wave elastography (SWE). A total of 51 participants (19 female, 32 male) were included in this study. Data including gender, age, blood group, height, weight, body mass index (BMI), body composition, International Physical Activity Questionnaire scores, and blood lipid values of the participants were recorded. Participants were divided into two groups according to their low-density lipoprotein (LDL) levels as study group (hyperlipidemia, n=24) and control group (non-hyperlipidemia, n=27). The patellar tendon and rectus femoris muscle thicknesses were measured by B-mode sonography, and patellar tendon and rectus femoris muscle shear wave velocities (SWV) were measured by SWE. Compared to the control group, participants with hyperlipidemia were older (27.9 ± 7.6 and 35.2 ± 10.9, respectively, p=0.006), whereas there was no significant difference between the two groups in terms of other demographic characteristics. Patellar tendon SWV was 5.02 ± 0.78 m/sec in the control group and 5.98 ± 1.19 m/sec in the hyperlipidemia group, and the difference was statistically significant (p=0.001). Patellar tendon SWV was found to be higher in overweight-obese subjects compared to normal-weight subjects (5.79 ± 1.19 m/sec, 5.18 ± 0.94 m/sec, respectively, p=0.042). There was a positive moderate statistically significant correlation between patellar tendon SWV and LDL (r=0.432, p<0.002). In the multiple linear regression analysis, which was used for evaluation of independent predictors of patellar tendon SWV, only LDL was found as a significant (CI: 0.005-0.028, p=0.007). In conclusion, we evaluated the effects of systemic hyperlipidemia and BMI on patellar tendon mechanical properties with SWE, which is a new diagnostic modality. We found that the blood LDL level had an impact on patellar tendon stiffness independently of BMI.
