Kalp Yetmezliği Hastalarında ve Sağlıklı Kişilerde Denge ve Yürüme Parametrelerinin Karşılaştırılması

. Heart failure affects most of the body systems and causes various problems in patients. Therefore, balance and gait parameters may be affected in patients with heart failure. The aim of this study was to evaluate balance and gait parameters in patients with heart failure compared with healthy people. 22 patients with heart failure with a mean age of 59.00 ± 2.58 years and 22 patients with healthy subjects with a mean age of 59.45 ± 6.8 years was included in the study. The physical activity level of the individuals was determined using International Physical Activity Questionnaire (IPAQ), quadriceps muscle strength and hand grip strength using hand dynamometer, exercise capacity were assessed using six minute walk test (6MWT), activity-specific balance confidence, using Activity-Specific Balance Safety Scale (ABC) and cognitive function, using Mini Mental State Examination (MMSE). Balance was evaluated with the Mini-BESTest balance battery. Gait analysis was evaluated with Biodex Gait Trainer. As a result of the study, right quadriceps muscle strength and dominant hand grip strength, 6 MWT distance, ABC Scale score, MMSE score were significantly lower in patients with heart failure compered to the healthy people (p<0.05). There was no difference in physical activity levels between the groups (p> 0.05). Total Mini-BESTest score and reactive postüral responses and gait stability sub-parameter scores were significantly lower in patients with heart failure compared to healthy subjects (p <0.05). Mean walking distance, mean step cycle, mean step length (right-left), coefficent of varriation (right) and ambulation index parameters were significantly different between the two groups (p <0.05). In conclusion, balance levels, mean walking speed, and distance, average step length and ambulation index of patients with heart failure were lower than healthy subjects. It is important to plan exercise programs including balance and walking training in patients with heart failure and to be included in the rehabilitation program. Thus, balance disorders can be eliminated and falls can be prevented in this patient group. Our study is leading and guiding the clinical trials that will be conducted on this topic.
