Serebral Palsili Olguların Postüral Kontrol ve Reaksiyonlarının Değerlendirilmesi
Özet
Postural control deficiency is one the main problems in cerebral palsy (CP). Continuity of the postural control has critical importance for achieve daily life activities. This study was planned to determine the postural control responses? differences between children with spastic type CP who were level I-II at gross motor function classification system and healthy children and to investigate detailed the postural control and reactions in children with spastic CP. Nine hemiplegic, 10 diplegic and 10 healthy children between the ages of 5-17 were evaluated. The Gross Motor Function Measure, Timed Up and Go Test (TUG), Timed Up and Down Stairs Test (TUDS), Pediatrical Berg Balance Scale (PBBS), Trunk Impairment Scale (TIS), Computerized Dynamic Posturography Sensory Organisation Test (SOT) were applied and range of movement of joints were evaluated passively to all participants. In addition, spasticity level of children with CP were evaluated. There were differences between CP group and healthy group in TIS, PBBS, visual and composite balance score of SOT significantly (p<0,05). But there were no differences at vestibüler and somatosensory scores of SOT. There were not any differences between hemiplegic and diplegic children in these tests. Children with spastic CP had more postural sway then healthy children (p<0,05). In children with CP, there were significant relationship between spastisity, joint limitation and trunk control and motor function capacity and composite balance score (p<0,05). When hypotheses identified for this study, threre were differences between children with and without cerebral palsy; but there were no differences between children with cerebral palsy but have different limb distribution. As a result, detailed evaluation of postural control responses and effect reasons are important to decide ideal physical therapy and rehabilitation program.
