dc.description.abstractReconstruction of the contour and functional deficiencies due to the absence of the bone tissue is among the most popular subjects Plastic Surgery deals with. In reconstructive surgery, autogeneous bone graft is considered as the best material to replace the damaged bone tissues after trauma or surgery. However, factors such as donor site morbidity, prolongation of operating time, difficulty in sculpting and the limited amount of donor tissue especially in pediatric population restrict the usage of autogenous bone grafts. Thus, biomaterials have become an integral part of the reconstructive surgery of the bone. Shorter operating time, ease of use and high resistance make biomaterials even more important. The primary aim of our study is to form a new bone (neo-osseous) tissue in the body, and to give extra volume and resistance to our newly formed bone tissue by using the biomaterial. In this study, 32 Wistar Albino Rats, supplied by the Hacettepe University Experimental Animals Unit, and which weigh 250-300 gr, were used. The biomaterial of choice to be used for the prefabrication of the flap was BonAlive® (BonAliveBiomaterials, Turku, Finland) bioactive glass, composed 53 % of silica dioxide, 23 % of sodium oxide, 20 % of calcium oxide and 4 % of diphosphopentaoxide. The subjects were allocated into 4 groups made of 8 subjects: Group 1 (FP) was the periosteum on the flap group, Group 2 (FPB) was the periosteum and bioactive glass on the flap group, Group 3 (FB) was the bioactive glass on the flap group, and Group 4 (F) was the flap-only group. Using 3 dimensional radiomorphometric analyses, the volumes of the areas with bone density were compared to the total flap volume in the subjects. The difference between the FPB and FB groups was statistically significant. Histologic examination revealed that lamellar bone formation was highest in the FP group, with a percent of 62,5 %, significantly higher than other groups. Also, according to the histological scoring, FP, FPB and FB groups all had a statistically important higher score compared to the control group.tr_TR

dc.description.ozetKemik doku eksikliğine ve yetersizliğine bağlı oluşan kontür ve fonksiyon bozukluklarının rekonstrüksiyonu Plastik ve Rekonstrüktif Cerrahi'nin uğraştığı konuların başında gelir. Travma veya cerrahi sonrası hasara uğramış doku ve organların yerini almak üzere, rekonstrüktif cerrahide, otojen doku en iyi materyal olarak kabul edilmektedir. Bununla birlikte donör saha morbiditesi, ameliyat süresinin uzaması, şekillendirmede zorluk ve özellikle pediatrik yaş grubunda sınırlı donör alan olması otojen kemik grefti kullanımını sınırlamaktadır. Biyomateryaller kemik rekonstrüksiyon cerrahisinin vazgeçilmez bir parçası haline gelmiştir. Kısa ameliyat süresi, kullanım kolaylığı ve yüksek direnç biyomateryallerin önemini artırmaktadır. Çalışmamızın amacı öncelikle vücutta yeni bir kemik 'neo osseus' doku oluşturmak ve oluşturmuş olduğumuz bu kemik dokuyu biyomateryal ile beraber kullanarak flebimize ekstra hacim ve direnç kazandırmaktır. Çalışmada Hacettepe Üniversitesi Deney Hayvanları Laboratuarı'dan temin edilen 32 adet 250-300 gr ağırlığında Wistar Albino Rat kullanıldı. Flep prefabrikasyonunda ise %53 silika dioksit, %23 sodyum oksit, %20 kalsiyum oksit ve %4 difosfopentaoksit yapıda olan BonAlive® (BonAlive Biomaterials, Turku, Finland) marka biyoaktif cam kullanıldı. Deney hayvanları sekizer hayvanlı 4 gruba ayrıldı. Grup 1 (FP) flep üzerine periost grubu, Grup 2 (FPB) flep üzerine periost ve biyoaktif cam grubu, Grup 3 (FB) flep üzerine biyoaktif cam grubu, Grup 4 (F) sadece flep grubu olarak tasarlandı. Yapılan 3D radyomorfometrik analizler sonucu örneklerde kemik dansitesinde olan hacim tüm flep hacmine oranlanarak değerler alındı ve istatiksel olarak FPB ile FB grupları arasında anlamlı fark bulunuldu. Histolojik incelemede özellikler lamellar kemik oluşumuna bakıldığında % 62,5 ile en yüksek oranın FP grubunda olduğu ve diğer gruplara kıyasla fark yarattığı görüldü. Histolojik skorlama dikkate alındığında istatiksel olarak FP, FPB ve FB gruplarının kontrol grubuna istatiksel olarak anlamlı fark yarattığı görüldü.tr_TR
