Abstract
In the primary or secondary prevention of sudden cardiac death, implantable cardiac defibrillators (ICD) are used as standard therapy. Purpose of the research; To examine the effect of upper extremity endurance training applied in addition to lower extremity endurance training on functional capacity and ICD parameters (shock and antitachycardia pacing) in heart failure patients with ICD. Twenty-four individuals who met the inclusion criteria were randomly divided into two groups. Demographic information, medical history and ICD parameters of the participants were recorded. Exercise capacities were evaluated with cardiopulmonary exercise test, arm ergometer test, and 6-minute pegboard and ring test (6PBRT). Both groups participated in 40 minutes of lower extremity training at 70-80% of peak oxygen consumption for six weeks, 5 days a week. The combined training group was also included in upper extremity endurance training for 20 minutes at 60% of peak workload. The peak oxygen consumption after exercise training was 1.58±1.37 ml/kg/min in the combined training group and 0.00±1.80 ml/kg/min in the lower extremity training group (p<0.05). Arm ergometer peak workload after training increased by 16.66±13.02 watts in the combined training group, while it increased by 4.16±7.92 watts in the lower extremity training group (p<0.05). In the 6PBRT test score, there was an increase of 16.66±21.24 in the combined training group and 7.83±14.23 in the lower extremity training group (p>0.05). When ICD parameters were examined, antitachycardia pacing was detected in only one person from the lower extremity training group. In conclusion, combined exercise training involving both extremities in heart failure patients with ICD was found to be effective and safe for the improvement of functional capacity.
