Cad Cam ve Geleneksel Yöntemlerle Üretilmiş Iki Farklı Tabanlık Uygulamasının, Ağrılı Esnek Düz Tabanlı Bireylerde Ağrı, Yaşam Kalitesi ve Fiziksel Performans Üzerindeki Etkilerinin Karşılaştırılması

Our aim was to compare two different type of insoles which were producted by Cad/Cam and conventional methods on pain, quality of life and physical performance in painful flexible flat foot. 67 people were randomized into three groups; Cad/Cam (n = 22), conventional (n = 22) and plasebo control (n = 21). Visual Analog Scale and Foot Function Index were used to investigate foot pain and Short Form-36 for quality of life at first and after two months. Balance, vertical jump and physiological cost index measurements were used to investigate physical performance with and without insole at the and of treatment. Also insole satisfaction was investigated with Visual analog Scale and a home exercise program was given for all groups. There were no difference at any values between groups before treatment (p > 0.05). There was significant reduction at foot pain and improvement at physical quality of life for all groups (p < 0.05). Pain reduction was significantly higher for intervention groups (p < 0.05). We found no difference between intervention groups at any measurements (p > 0.05). Conventional method got higher satisfaction than plasebo (p < 0.05). We did not found any effect of insoles on balance, vertical jump and physiological cost index (p > 0.05). Only plasebo showed an improvement in vertical jump but the difference was minimal. Our results show that orthotic treatment is an important part of treatment in painful flexible flat foot. Both Cad/Cam and conventional insole types can be prescribed. When side effects of pharmalogical and surgical options are taken into consideration these results of conservative therapy get more valuable. In future studies, to improve effect of treatment a comprehensive physiotherapy and rehabilitation program with a corrective insole could be more effective.
